Law Enforcement Crash Reporting

In the event of a crash, Law Enforcement agencies have two options for reporting the incident: Paper Reporting and Third Party Submittal. Please see below for more information on these reporting options.

Paper Reporting

The SR 1050 is the only crash report form approved by the department, as the designated Administrator of crash information for the State of Illinois, for use in reporting crash investigations to the Office of Planning and Programming's Bureau of Data Collection. No other crash report form is authorized. Traffic Crash Report forms are printed and furnished by the IDOT's Bureau of Data Collection.

It is extremely important that all required fields on the SR 1050 be completed accurately, completely, and legibly. The SR 1050 manual has been created to assist local law enforcement agencies in filing out the forms.

Third Party Submittal

To reduce crash-related injuries and fatalities in Illinois, it is vital that the department receive crash reports and data as quickly as possible. By processing this data in a timely manner, the department can ensure it is complete, accurate and available for statistical analysis, research and decision support. In the last 10 years, the overall injury and fatality counts have been declining in Illinois, but there is still more work to be done to continue that trend. The department is assisting law enforcement agencies by partnering with third-party crash reporting vendors to electronically submit the data that is captured in their reporting systems.

In an effort to work with state and local law enforcement agencies and software vendors, a new program has been implemented allowing submission of crash report data electronically through third-party XML vendors. The department defines the XML format and tests third-party vendor’s software before agencies are allowed to use their systems to submit crash reports electronically. Crash data is validated upon submittal, and if accepted successfully, a department authorized bar code number is assigned to each crash report. A number of third-party XML vendors have been approved to participate in this program. However, it is very important to note that the department will not accept electronic reports via third- party vendors that have not participated in the established registration and data verification process, designed to ensure that electronically submitted crash data meets a minimum level of quality.

The department encourages all law enforcement agencies to begin to move forward from paper submittals to electronic crash reporting.

  2. IDOT will create a Non-Disclosure Agreement (NDA) and send it to the vendor to be reviewed, signed and returned.
  3. Once the NDA has been returned to IDOT, a conference call will be setup with the vendor to discuss the approval process and the specific technical requirements of the XML transfer.
  4. IDOT will work with the vendor to implement the requirements and will assist with the testing of the vendor's XML submittals.
  5. Once the vendor has successfully completed the testing of its XML transfer, it will be added as an approved Third Party XML Vendor.

IDOT's Division of Transportation Safety is the administrator of the state's traffic crash information, as set forth in the Illinois Compiled Statutes: 625 ILCS 5/11-401. As Administrator, IDOT is required to (1) tabulate and analyze motor vehicle crash reports, and (2) ensure that every crash report is submitted on an appropriate form approved or provided by IDOT. Since 2001, there have been only two crash reporting forms approved by the department: the paper SR 1050 Illinois Traffic Crash Report form, and the electronic-based Mobile Capture and Reporting (MCR) form. Recently, a third format for submitting crash reports to IDOT has been approved: submitting via an XML format defined by IDOT.
