GUIDELINES
REQUIRED DOCUMENTS

Once this new link is clicked or selected by the grantee, the following documents will appear for Infrastructure applications only. The following links to appropriate documents (LD) and messages (M) are required within new “Infrastructure Application”:
- Current Illinois SRTS Funding Application – Infrastructure (PRO SRTS 2300) (LD)
- Student Tally List (LD)
- Parent Survey English (LD)
- Parent Survey Spanish (LD)
- BoBs 2831: Uniform Grant Agreement Affidavit of Disclosure of Conflicts of Interest – Grantee (LD)
- Detailed Cost Estimates infrastructure project (BDE 213)(LD)
- Documents Assuring Financial Commitment (Government Resolution) (M)
- Resolution or Letter of Support from the School District (M)
- Letter of Allowance by IDOT district office if project is located along a state route (M)
- Any other maps/documents/photos that help to present your project (M)
NON-Infrastructure Application
